Near the coast, salt is a real-ish consideration on exposed blocks. What the work involves

Should there be an overflow?
On a box gutter, yes. If the primary outlet blocks and no overflow exists, the water has one remaining route and it goes through your ceiling. Adding one converts a catastrophe into a visible nuisance.
Is roof cement ever acceptable?
Not as a repair. It is rigid, the junction it covers moves and it cracks. Worse, it traps water behind it and conceals what that water is doing.
